UCare offers two plans for people ages 18 to 64 with certified disabilities:
- UCare Connect + Medicare is designed for people who want to combine their Medical Assistance (Medicaid) and Medicare under one plan.
Order a kit orenroll now - UCare Connect is created for people who qualify for Medical Assistance, but do not have Medicare (or choose to keep their Medicare under a separate plan).

About UCare Connect + Medicare
UCare Connect + Medicare
UCare Connect + Medicare provides health care coverage to Minnesotans with certified disabilities. It combines the benefits and services of Medicare and Medical Assistance.
Eligibility
You are eligible to join UCare Connect + Medicare if you:
- Have a certified disability or receive waiver services for your disability from your county
- Are at least age 18 and under age 65
- Have Medical Assistance and Medicare Parts A and B
- Live in the 62-county UCare Connect + Medicare service area

- No-cost transportation to and from covered medical, dental and pharmacy appointments through UCare Health Ride
Focus on wellness
- No-cost SilverSneakers® Fitness gym membership at all participating fitness locations, visit SilverSneakers.com for more information. Round-trip transportation 3 times per week
- No-cost fitness kits with at-home activities such as Stress Relief, Tai Chi, Sit & Be Fit and Latin Dance upon request
- Up to a $15 discount on most Community Education classes in Minnesota (limits apply)
- Save up to $50 a week at participating grocery stores with Healthy Savings. Shop and save when you buy select healthy foods including milk, whole-grain bread, lean meat, eggs, yogurt, fruits, vegetables and more.

- Dental care aboard the UCare Mobile Dental Clinic when it’s in your area
- UCare 24/7 nurse line, so you know where to go when you’re not feeling well
- Personal medication review by a pharmacist after an inpatient stay so you understand and use your medications safely and correctly

Keep Your Coverage Program
As a UCare member, you may occasionally need to complete and return Medical Assistance renewal forms sent to you by your county. Below are a few resources for completing your renewal.
- List of county phone numbers and addresses (external site) - Your county is a great place to start when you have MA renewal questions.
- Disability Hub MN (external site) - Metropolitan and Southeastern MN Centers for Independent Living offer up-to-date information about community resources related to people with disabilities, including housing and benefits referral.

About UCare Connect (Special Needs BasicCare)
UCare Connect (Special Needs BasicCare)
UCare Connect is a way to access the benefits you are eligible for through Medical Assistance, which is Minnesota’s Medicaid program. You don’t have to pay anything to join the plan, and there is no deductible you have to meet before the plan starts paying for covered services. The health plan also offers additional benefits not available on regular Medical Assistance.

Coordinating your care
When you join UCare Connect, a team of health care professionals will be dedicated to helping you meet your health and wellness goals. You will be assigned a Care Navigator, who can help you:
- Identify or change your primary care clinic, or primary care provider
- Learn about preventive care, such as annual physical exams, vaccinations, and screenings
- Enroll in a disease management program if you have asthma, diabetes, or heart failure, and you meet the program criteria
- Get information regarding programs so you can start exercising, quit smoking, prepare for a baby, or take other steps to improve your health
You may be offered comprehensive case management based on your level of need.
